Every Oregon DMV branch location is supposed to maintain a binder with sample SR22 forms from each auto insurance company doing business in Oregon. Inside that binder, the representative can thumb through the pages to the company and compare the certificate for authenticity. Fortunately, my client found someone patient enough to look it up and recognize that the certificate in his hand matched the format they had on file, and she accepted the form and reinstated his driver’s license.

Please be patient with Oregon DMV associates. They have a lot of responsibility and are tasked with everything from the licensing of drivers to the registration of vehicles. Keep your cool and ask to speak with a supervisor if the SR22 certificate I give you has been rejected. I’ll be happy to talk you through the steps to get it approved.
